The primary goal of CTC Wounded But Not Broken, Incorporated is to focus on habilitation not rehabilitation. To provide and support the veterans in becoming productive, functioning and contributing members of their perspective communities, providing support whenever possible for community participation as they choose and are capable. The best holistic approach is for the veteran to be integrated into, rather than isolated from the community.
The Program will offer an open door policy; allowing Veterans the opportunity to quickly access support and services as needed; avoiding required appointments and long wait times to access services. CTC Wounded But Not Broken will provide on-site Case Managers to assist Veterans with their needs. The program will address needs related to food, shelter, access to personal care items, assistance with the medication administration, and linking Veterans to other needed resources and services in the community. Case Managers would make referrals for Health, and Mental Health Services as needed.
The Program will be operated in the states of Texas and Oklahoma identified as a Case Management Program. The program will be a place for the veterans suffering from medical, mental and emotional challenges can find a safe and secure place in the community to receive support and services.
CTCWBNB will be a highly structured program, which will provide linking Veterans to daily living skills training, functional living skills, recreational activities, food, shelter, Mental Health and Heath Care Services. The Program will provide a 24-hour access to support and services of highly trained Case Mangers. Our goal is to assist each Veteran to continue being a contributing, self-sufficient, and productive citizens.
CTCWBNB will be a Case Management Program (prototype from Minnesota Department of Human Services), designed to meet the various yet unique needs of the veterans. The Case Management Program will help the veterans to gain access to needed medical, social, educational, vocational, and other necessary services as they relate to the veteran’s mental health needs. The program with include developing a functional assessment and personal development strategy, referring and assisting the veteran in obtaining needed mental health and other services, ensuring coordination of services and monitoring the delivery of services.
The eligible recipient for these services will be any veteran identified by the Veteran Health Administration who are suffering with PTSD and TBI.
